1. Acceptance of Terms

By accessing, using, or submitting information to UnchainedDentistry.com or any related services (including the Unchained Dentistry Directory and CTAKode tools), you agree to be bound by these Terms of Service. If you do not agree, you must discontinue use of the site and request removal of your listing.

These Terms apply only to users and dental practices located in the United States. We do not list or support non-U.S. dental practices.

2. Description of Service

UnchainedDentistry.com provides:

A public directory of independent, privately owned dental practices

Optional paid promotional upgrades, including Premium listings

A free website engagement tool (“CTAKode”) offered to verified listings

Email and optional SMS communication related to listings and onboarding

Educational content and media related to private-practice dentistry

We do not provide dental services, patient scheduling, clinical advice, or insurance processing.

3. Eligibility

To be listed in the Unchained Dentistry Directory, a practice must:

✅ Be a privately owned, independent dental practice (not owned, funded, or controlled by a Dental Service Organization/DSO, private equity group, or corporate chain)
✅ Be physically located and operating in the United States
✅ Not misrepresent ownership, licensing, or credentials

Listings may be removed without refund if a practice is found to be:

Fully or partially owned by a DSO, corporate entity, or non-dentist investor group

Misrepresenting ownership, licensing, or professional identity

Providing fraudulent, misleading, or illegal content

This “Independent-Only” rule is core to the mission of Unchained Dentistry Alliance.

4. Directory Listing Types

We reserve the right to modify listing tiers and features at any time.

5. Paid Listings, Billing, and Refunds

Premium listings are billed yearly at $299 (USD) unless otherwise stated.

Subscriptions auto-renew unless canceled before renewal date

Refunds may be requested within 30 days of initial purchase

No refunds are issued after renewal unless required by law

Payment is processed securely through a third-party provider

If a Premium listing is removed due to DSO ownership disclosure or terms violation, refunds are not guaranteed.

6. Accuracy of Information

It is the responsibility of the dental practice to ensure all information displayed in its listing is accurate and up to date.

By submitting updates, you confirm you are authorized to act on behalf of the practice.

We are not responsible for listing errors resulting from:

Outdated public data sources

Third-party scraped or imported content

Lack of response to verification requests

Practices may request updates at any time.

7. Communications

By submitting a form, verifying a listing, or upgrading to Premium, you may receive:

Transactional emails related to your directory listing

Optional educational or marketing content (unsubscribe link included)

Optional SMS or AI-assisted voice calls only if you opt in

You may revoke communication consent at any time by emailing [email protected] or replying STOP to text messages.

8. CTAKode Tool

CTAKode is a free web-to-mobile button dock provided to Verified and Premium practices.

We provide:

Setup assistance

Optional onboarding guidance

Support via email

We do not guarantee specific lead volume, review count, or revenue impact.

Practices are responsible for maintaining compliance with local advertising laws, ADA rules, and state dental board guidelines.

9. Prohibited Uses

You agree not to:

❌ Use the directory to solicit practices for commercial advertising, recruiting, or list building
❌ Misrepresent ownership, credentials, or licensing
❌ Scrape directory data for resale or unauthorized use
❌ Attempt to bypass paid listing features with manual edits or submissions
❌ Submit or upload patient health information (PHI)

10. Intellectual Property

All site content, brand assets, logos, and visual elements are property of ExpressPlan, Inc. unless otherwise noted.
You may not copy, duplicate, or redistribute without permission.

You may link to your own listing, embed your listing badge, or reference your presence in the directory for marketing purposes.

11. Disclaimers

We are not a healthcare provider or referral service

We do not endorse or guarantee the quality of care of any listed practice

No medical advice is given or stored on the platform

Listings do not imply clinical certification, patient safety, or insurance participation

This website is provided “as-is” without warranties of any kind.

12. Limitation of Liability

To the maximum extent permitted by U.S. law, we are not liable for:

Loss of revenue or reputation

Listing removal or downtime

Errors or omissions in listing content

Third-party platform failures (CRM, hosting, payment processor)

Indirect, exemplary, or consequential damages

Maximum liability shall not exceed the annual subscription fee paid, if any.

13. Termination

We may suspend, edit, or terminate a listing at any time for:

Policy violations

DSO/corporate ownership discovery

Abuse or harassment of team members

Illegal, fraudulent, or deceptive activity

Practices may request voluntary removal at any time.

14. Governing Law

These Terms are governed by the laws of the State of Georgia, United States.
Any disputes will be handled in courts located in Georgia unless otherwise required by law.
